[Skin concerns] How do I fix the texture of my t-zone?

My t-zone is super problematic and permanently looks like this: http://imgur.com/DLaPPfvThese bumps are flesh coloured but very visible from the side as they protrude. Occasionally one will flare up and become a full blown pimple but once it heals it still leaves a flesh coloured bump.I use concealer daily on this area to cover up the bumpiness and my skin care routine is basically just cleanser and moisturiser once in morning and once at night.Please help me get rid of this :(!
