Perhaps a controversial topic and it may not be the cause for everyone but since I was 15 I have had acne problems mostly on my chest, nose and chin. A few months ago I stopped taking any form of dairy and within a few weeks everything cleared up. Without any other changes to my diet I added dairy back again recently and of course slowly spots started appearing again so I am quitting dairy indefinitely. Whey, milk and cheese seem to be the worst culprits. I also remember the worst acne on my chest between ages 16 and 18 back when I was drinking at least 1 full glass of milk a day. I had started a basic skin care routine over a year ago and still continue with it but the effect on spots is not really noticeable. There seems to be some research and similar stories to back this up if you google 'Dairy acne' or something similar. Try it yourself and see if it helps. Expect to wait a few weeks before noticing any results (if any).
