[Acne] - Please Help

**Note: there is an imgur gallery at the bottom. What type of acne is this?I am 18 years old and in my last year of high school. I have had acne since seventh grade, but my worst bouts of acne have been over the last three years. I have been to the dermatologist twice - both times he recommended a routine to follow: wash face with cleanser in shower followed by moisturizer. I followed his specific instruction until I started to get massive cysts on my cheeks. I continued to use his specific cleanser/ moisturizer for six months until I pulled the plug after it was clear my face was significantly worse.I have stopped eating dairy (no whey powder, no cheese, no milk), I have stopped eating chocolate, I have stopped drinking soda, and I have tried to eliminate fast food as much as possible. I do not drink, smoke, or do any drugs. I don't drink coffee or energy drinks. I change my pillowcase every night to help with the acne. I don't touch/ pick on my face at all.I work out (lifting heavy four times a week) and I run twice a week as well. I sleep as much as I can (avg 7 hours on school nights) and I try to keep stress to a minimum.I have tried many antibiotics, as well as creams such as tretinoin and benzoyl peroxide. I have tried salictic acid, but nothing has worked. This was all during freshman/ sophomore year and my acne was constantly getting worse.HOW I GOT RID OF MY ACNE FOR SIX MONTHS: Eventually, it got to the point where I would have ten to fifteen massive papules at a time and they were leaving deep, red hyper pigmentation. Last year I decided to take the drug accutane to alleviate my acne. Accutane did work, however, it dried my skin out and I felt extremely lethargic and borderline depressed on it. It did an amazing job clearing my face, chest, and back, but it was impossible to continue.I was acne-free from March of this year to around Halloween. Ever since I have stopped using Accutane (February) I have used absolutely no cleansers/ moisturizers. It may seem stupid but I only washed my face with water and there were no problems for six months.Here are my possible causes of acne:1) Genetics - my dad had acne, but not nearly as much as I have now. I also have oily skin.2) Testosterone/Hormone Inbalance - When I work out/sweat I notice an increase in acne. Is it possible that my hormone glands are messed up somehow?3) My doctor said there is a correlation between wisdom teeth coming in and an increase in acne. Anyone have any experience with this?4) Stress - Once I got back in school, stress became a daily factor. College applications, exams, and general deadlines add stress everyday.5) Shampoo/conditioner - I know this is a stretch, however I believe that my shampoo may be causing my acne, but I don't know what specific ingredient is responsible.6) Pillowcase - I change my pillowcase every night. It night be possible that the detergent that is used on it might irritate my skin.7) Allergies - I am allergic to dust and freshly cut grass; can this somehow lead to my skin being irritated and lead to acne breakouts?9) Thyroid problem/ diabetes - Something my dermatologist mentioned. Maybe I am suffering from some type of disease?To summarize quickly: I don't have any skincare routine right now. I know I have wrote a lot, but please read through it all to have a better understanding on how persistent this disease is. If you can help me, please PM me or leave a message. I am seriously struggling here and would appreciate the help.TLDR: Even after Accutane, acne has come back. I don't have any products right now.
