[Acne] I've never had normal skin on my back and I'd like to fix that

Ever since puberty I've suffered from this stuff on my back (and my chest).I don't know what it is. The large red bumps are extremely painful and 50% of the time they will come to a head. If I squeeze them, there will be some white discharge/fluid along with a large white chunk afterwards if I squeeze hard enough. Then the bump will stay red for weeks and slowly recede but still be noticeable. The other 50% of the time they will stay red and painful and eventually the pain will go away, yet the bump and redness will remain forever.I also have small red painless blotches that are 50% bumpy and 50% flat that seemingly come and go.I don't get it anywhere else on my body and it's pretty frustrating because I consider myself v hygienic. I exfoliate when I shower and I shower every night before bed.Can someone point me in the right direction here?
