[Acne] Is a whitehead considered a whitehead if there's pain when touched? Or, is it a cyst then?

I hear that whiteheads are painless, and that if there's pain, it means there's inflammation deep in the skin layer close to a nerve which makes it a cyst. Does that mean these painful pimples are actually cysts and are more likely to scar?
