[ACNE] I haven't had clear skin since I was 12. I'm 23 now.

I'm getting really tired of my acne to the point where I'm starting to give up. When I first started getting acne it was really bad, cystic and I had it all over my face, neck, chest and back. I was told it would go away after puberty, but it didn't. It's cleared up a little bit but I still can't remember the last time I was pimple-free even for a day. The scars and occational cystic pimples on my chest and back also make me really self conscious about wearing low cut tops or a bikini. It's sad because I'd really like to go to the beach this summer but I'm too nervous to do it. I've tried birth control for acne (only made me gain weight) and Tetracycline which worked but the acne came back worse once I stopped.I've tried to talk to a doctor about trying Accutane but she said it was too risky and that I should do what I can on my own instead. I drink a lot of water, I've cut out dairy and found a good skincare routine after years of trying literally everything, but the acne is still there. I'm starting to think there could be a deeper underlying issue, like my thyroid or hormonal imbalance? It's so hard to get proper medical help where I live, though. Like they don't listen. I always get the same advice from doctors but it never works. Does anyone here have any advice or have a similar problem? Have you found something that works for you? This whole thing really messes with my confidence and I'm so sick of it.
