[Acne] I have no idea what this condition is or how to get rid of it

http://imgur.com/a/Vbj36I have combination skin and the area in the photo is my oily zone. What's this skin condition called? I've had this weird acne on my face for many years, ever since puberty. It doesn't quite look like milia to me but if it were, there's no way I can extract ALL of that.I've tried using a clarisonic to no avail. I've tried exfoliating with face washes with microbeads, but that broke me out.Please help me out guys?!
